Actually, it doesn't hurt (Well, of course you can feel it, but it isn't anywhere close I expected). When I enlarged it, it hurted more than the first time. Only unpleasant sensation was when she (Yes, a girl made it for me... ) put "safety cylinder" to urethra to prevent through-and-through.
