All the incidents in this post are from Practice Session 4 RACE:
Warning to Car 999 to assure clear room when moving in the formation laps.
Car 9 is ordered to figure out a way to be in TeamSpeak during the session - the driver should have been prohibited from taking part, but I waived that regulation for this case.
No action against Cars 27 and 606.
Car 21 - Warning for causing an avoidable incident with car 87.
Car 8 - Warning for failure to react to a visible incident ahead.
Cars 6, 9, 87, 96: No action.
In-Race, Car 30 was issued a stop-go for causing an avoidable incident, and retired in pitlane when his damage wasn't repaired, prior to serving the penalty. The driver will instead be assessed a 1-session deduction.
Cars 8, 21, 87, 96, 420: No action.
Drive-Through Penalty for overtaking before the green flag. While you may hear me call it before the message, the in-game text messages with the track status are the official point where the track status changes. The driver was issued a stop-go for speeding in the pitlane while serving the DT. Due to cautions preventing him being able to serve the SG; all penalties relating to this incident are considered served.
Car 61 - Warning - Make sure room is left for car(s) on the inside. Car 21 was leaving extra room due to the car to his inside.
Cars 21 and 999: No action.
Car 420 - Warning (Would be a DT penalty in the race proper) for causing an avoidable incident. Car 18 selects the low-line exiting Turn two, as he is entitled to do, then moves up about one car-width to take Turn 3, which is also allowed - neither move is sudden or unsafe. Car 420 tries to slip in this gap which isn't really a gap. It is advised to all drivers to be judicious for most of the laps, and not try for gaps that "may" exist.
Yes, is Jump Start. Car 18 lays several car lengths back, and begins the process of overtaking a car before green flag. Warning to Car 18.
Car 61 punts Car 21, probably through not paying attention. Accidents on the cool-down lap are unprofessional and not tolerated. Car 61 excluded from session for causing this incident.
Also on the cool down lap: Car 87 excluded from session for causing avoidable contact with Cars 6, 18, 36. NOTE: Some contact when turning into a garage is allowed, but frowned heavily upon.
Missed during session, so Car 77 will only be warned for this occurence of the infraction.
On the same restart; several cars (including but not limited to 37, 73, 420 - in that area of the queue) seemed to try to time the start and nearly caused a significant accident. Just remember that you need to be careful with that sort, and I'd rather you commit the overtake and get a DT than slam on the brakes to avoid it and risk a massive shunt.
Car 19 - Excluded from session - Avoidable contact with Car 420 during cool-down lap
Car 27 - Excluded from session - Avoidable contact with Car 73 during cool-down lap.
All drivers are reminded that the cool-down lap is to be taken at reduced pace, and contact is to be avoided. Please consider yourself under caution once you've finished the race - contact is unnecessary and always avoidable on the cool-down lap.

Brief Description of incident 36 pushing me off, leaving nowhere near enough room for me, causing me to spin out and lose the race
Warning to Car 36 for failure to assure clear distance to competitor.
A compliment to all behind the race start incident in Session 11 - great avoidance for those who were able to - those caught were just unlucky. Not severe enough to issue a penalty for the cause, but a reminder that on the start you're close and need to be ready to lift just enough to avoid contact - people react to the green at different rates!
All cars made the required pitstops, excepting Car 64 who did not change all tyres on the second stop. Deduction of one credit from the total awarded to him for the session.
Car 19 does not leave sufficient room for Car 77 on T1 entry, creating an avoidable incident. No session credit for Car 19.
Lag. Car 999 exhibiting high latency all session; unforunate accident causing heavy damage to Car 61. No action.
Cars 46 and 77 make contact crossing the start/finish line, and 41 gets caught up, and slides, and in turning the wheel to the right (if not to recover a slide the turn to the right is inexplicable) makes contact with the T1 wall, and rolls. Car 46 warned for failure to leave clear room for a car on the inside.
Car 999 latency-related incident. The other cars involved were largely unfortunate, although they are all reminded to begin slowing from full pace as soon as the caution is called.
