It knows because you're logged in. Just like it says 'Welcome, /you' in the top right corner of the page.

/me outputs the name of whoever posted it.
/you outputs the name of whoever is reading it.

eg, '/me gives /you a cheese sarnie' would show as 'Bean0 gives <reader> a cheese sarnie'.
